We have correlated as many accounts as possible but there are cases where an address or name doesn’t exactly match so we want to be sure we include everyone. Please fill out all fields. If you have any questions or comments, please add them to the form in the notes section.
As a side note: if you recently received an invoice that looks like the one below, that invoice shows the number of issues you are being credited for based on the amount of revenue remaining in your Archive subscription. In the case below, 5 additional issues were credited.
